This database was developed to allow the health and human service organizations who disperse EFSP funds to record those disbursements in order to comply with ESFP non-duplication requirements.
The following link leads to our EFSP Online New User Form. Best practices in regards to data security dictate that each user have a unique Username and Password. Please click on the link below to access this form and enter the Username and Password for each of your EFSP Online users. You may also use this form to request changes in Usernames and Passwords or to request the deletion of a Username.
